The Government of Yukon is seeking a physician to provide part-time services up to 15 hours per month for Integrated Health Services from July 2026 – March 31, 2027.  

As the population ages, primary care clinics are increasingly caring for older adults with complex medical, functional, and cognitive needs. Timely identification, assessment, and management of cognitive impairment, dementia, frailty, and associated geriatric syndromes are essential to improving patient outcomes, supporting caregivers, and enhancing quality of life.

To strengthen capacity within primary care, we are seeking expressions of interest from physicians with expertise in geriatrics, cognitive impairment, and elder care to serve as Physician Advisor. The Physician Advisor will provide clinical consultation to primary care providers and allied health team members, education, mentorship that supports interdisciplinary primary care teams in providing high quality care.

Program Description

Since its creation in 2021, Integrated Health Services (IHS) has launched or taken over the operations of five clinics/programs. Current clinical operations requiring consultation on elder care and cognitive impairment include the Centre de Santé Constellation Health Centre (Constellation), the Yukon Healthy Living Program (formerly Chronic Conditions Support Program), and the Whitehorse Walk-in Clinic.

• Constellation is a bilingual primary health care clinic serving Yukoners of all ages, from babies to older adults in both French and English.
• The Whitehorse Walk-in Clinic is an episodic non-urgent primary health care clinic serving Yukoners who do not currently have a primary care provider.
• The Yukon Healthy Living Program is a collaborative care team that offers individual and group support for people who have chronic conditions with nurse educators, exercise specialists, certified respiratory educators, dietitians, and pharmacists.

All of the clinics and programs are multi-disciplinary. Some examples of the types of staff and contractors include physicians, nurse practitioners, registered nurses, licensed practical nurses, exercise specialists, social workers, dieticians, pharmacists, and medical office and administrative assistants.

The successful candidate will collaboratively with IHS clinics and programs to promote evidence-informed best practices in the care of older adults, with a particular focus on:

• Early identification and assessment of cognitive impairment and dementia
• Comprehensive geriatric assessment principles
• Frailty screening and management
• Medication optimization and deprescribing
• Management of behavioural and psychological symptoms of dementia
• Care planning and caregiver support
• Advance care planning and goals-of-care discussions
• Appropriate referrals to specialist and community resources
• There are no direct practice or on-call requirements for this position. This position may have patient encounters while observing multidisciplinary team members for mentorship purposes.

Contract Details

Remuneration

• $200/hour

Shadow Billing

• Shadow billing not required
• Detailed, time-based invoices required

Start Date and Term

• Starting ASAP to March 31, 2027

Time Commitment

• Schedule and timing as agreed to with the clinic and program managers
• Monthly hours will vary

Contract conditions

• Ability to meet the following conditions are a contractual requirement and are required in advance of the contract start date.

Qualifications

Education and Licensing:

• Eligible for licensure with the Yukon Medical Council
• Maintain membership with the Canadian Medical Protective Association
• Holds a CAC for Care of the Elderly (CCFP (COE)/MCFP (COE)

Skills and Abilities:

• Demonstrated practice in geriatrics, family medicine with a focused practice in elder care, geriatric psychiatry, neurology, or a related speciality
• Demonstrated expertise in cognitive impairment, dementia care, and management of complex older adults
• Strong interpersonal skills with the ability and willingness to mentor and coach
• Demonstrated commitment to providing clinical education, mentorship, and/or quality improvement leadership
• Demonstrated commitment to high-quality care and continuous improvement
• Demonstrated knowledge of trauma-informed and anti-racist approaches, as well as a commitment to cultural safety and humility

Selection Process

Selection decisions will involve a review of all skills and abilities listed above. The selection process may include references and a meeting to discuss practice philosophy and fit within IHS operations.
