Centre de Santé Constellation Health Centre (CSCHC) is a bilingual primary health care clinic serving Yukoners of all ages, from babies to older adults in both French and English.
Providers work as a collaborative health care team, providing primary health care to a client panel, as well as consultation services.
The CSCHC is comprised of:
- a clinic manager
- nurse practitioners
- contract physicians
- registered nurses
- licensed practical nurses
- a social worker
- medical office and administrative assistants.
The CSCHC provides point-of-care services for clients, chronic disease management, care planning and case management, mental health counselling support and group visits.
