The Government of Yukon is seeking a physician to provide part-time services two days per week at Centre de Santé Constellation Health Centre from March 2025 – March 2026.
Centre de Santé Constellation Health Centre (CSCHC) is a bilingual primary health care clinic serving Yukoners of all ages, from babies to older adults in both French and English.
The successful candidate will work as a member of a collaborative health care team, providing primary health care to a client panel, as well as consultation services and case review support to Nurse Practitioners. The successful candidate is also responsible for supporting access to same-day appointments for all registered clients.
The CSCHC is comprised of a clinic manager, nurse practitioners, contract physicians, registered nurses, licensed practical nurses, a social worker, and medical office and administrative assistants.
The CSCHC provides point-of-care services for clients, chronic disease management, care planning and case management, mental health counselling support and group visits.
The CSCHC has and will continue to host medical residents, medical students, and nurse practitioner students.
The CSCHC is open 9am - 6pm Monday - Friday. There are no on-call requirements for this position.
